First Visit

An appointment letter with date and details and forms required to be completed will be posted/e-mailed to you prior to your appointment.

Please bring these forms and all your relevant imaging with you to the appointment. 

Please remember payment is required on the day.

You are welcome to bring a friend or family member to your appointment.

A referral to see Mr Simon Smith is required to initiate the appointment process. 

 

Referrals are available from your GP clinics and other medical specialists. 

• A GP referral is valid for 12 months. 
  
• A referral from a Specialist is valid for 3 months. 
  

Once the Referral is received in our Rooms, Mr Simon Smith will triage the Referral and an appointment will be posted/emailed to you. Please ensure the referral states if you are privately insured and your contact details are correct.
